This is a great text to understand how to document. I am a licensed psychotherapist and use this as a teaching resource to train new therapists and clinicians at an agency. A must have for every agency that trains associates/interns.

The book breaks clinician knowledge down, the author gives his learning tools, and some pitfalls to be alert and stay away from. An update for DSM 5 is needed.

The psychotherapy documentation primer provided basic information to assist in diagnosing clients. They provided a blueprint, which has basic symptoms counselors needs to insure appropriate diagnoses.

Perfect - and just what I was looking for. The writing clear and concise. The author handles the subject with ease. I've learned a lot from the book about documentation, diagnosis and treatment planning that I had not known before. I'm still reading the book but so far it has been a pleasure to read, professional and easy to understand.

These is an excellent book that was an eye opener for me. Taught other social workers in the hospital where I worked about what they were documenting and how to make it easier for managed care to access information they needed.
This book will probably cover everything you need to know about documentation.

Excellent book. I needed to improve my treatment plans and the book provides the information I needed. Also covers assessments, psychological evaluations, and other types of documentation. I like the little quizzes at the end of each chapter, and the sample documents.
Not a full documentation primer.

This documentation primer is not complete. It contains some basics (writing up a mental status; not very good content) and then focuses primarily on writing treatment plans for managed care. In that sense, for people who may not have had experience writing such treatment plans (and treatment plan updates), the primer is excellent. It does not really address many other aspects of documentation. In this way, some of the claims on the cover appear to be somewhat overstated.
